Join a progressive, well-run practice where the patient is a priority and a sensible working day still exists. This is a fixed term six-session GP role, covering maternity leave, within a supportive, forward-thinking team that values autonomy, good communication, innovation and calm, organised systems. You will work across our main and branch site, within a total triage model that gives GPs control over how they run their day. Clinicians triage their own patients and decide how best to manage them, whether that is face to face, by phone or digitally. Appointments are fifteen minutes and the day is structured but flexible. On-call duties are shared fairly across the team. We are EPS enabled, well staffed and focused on doing things properly rather than reactively. There is scope, for those who wish it, to become more involved in the longer-term development of the practice, including potential progression to a permanent role. About us
We are an established practice based in Ammanford, with a branch site in Glanamman. Our clinical team includes 8 GPs, a Physician Assistant, a pharmacy team, a Frailty Lead, practice nurses and HCAs. Job responsibilities

- Reporting to Senior Partner / Practice Business Manager
- To provide safe, effective and patient centred general medical services as part of the practice clinical team, working within a total triage model, undertaking house calls where required, and contributing fairly to on-call and service delivery.
- Deliver routine general practice consultations in line with GMC standards and contractual requirements.
- Work within the practice total triage system, undertaking telephone, digital and face to face consultations as appropriate.
- Participate in the GP on-call rota on a fair and shared basis.
- Undertake home visits as part of routine clinical duties.
- Manage patients with multiple long term conditions and complex needs, reflecting an older and more deprived patient population.
- Work closely with pharmacy, nursing and wider MDT colleagues to ensure appropriate clinical input and appropriate workload distribution.
- Maintain accurate, timely and high quality clinical records.
- Use practice clinical systems and digital tools in line with agreed protocols and workflows.
- Contribute to continuity of care and safe clinical decision making.
- Engage with clinical governance activity, safeguarding processes and quality improvement where required.
- Work collaboratively with the Senior Partner and Practice Business Manager to support safe and effective service delivery.
Person Specification
- Medical degree recognised by the General Medical Council
- Full registration with the General Medical Council with a licence to practise
- Completion of UK GP specialty training or equivalent
- Certificate of Completion of Training in General Practice or recognised equivalent
- Inclusion on the NHS England, NHS Wales, NHS Scotland or HSC Northern Ireland GP Performers List (as applicable)
- Right to work in the UK
- Evidence of ongoing revalidation with the General Medical Council
- Postgraduate qualifications relevant to general practice, for example PGCert, PGDip or MSc
- Additional qualifications supporting a recognised specialist interest, such as minor surgery, dermatology, women's health, diabetes or frailty
- Membership or fellowship of the Royal College of General Practitioners beyond MRCGP, where applicable
- Formal qualification or accredited training in teaching, mentoring or supervision
- Evidence of completed quality improvement or leadership programmes
- Training or accreditation relevant to extended roles, for example care home medicine or long term condition management
- Additional qualifications aligned with population health or deprivation focused care
Experience
- Recent experience working as an independent GP within UK general practice
- Experience managing undifferentiated presentations across all age groups
- Experience managing patients with multiple long term conditions and complex health needs
- Experience of working within NHS primary care systems and pathways
- Experience using clinical systems for consultation, coding and record keeping
- Experience contributing to on-call or duty doctor services
- Experience working safely and effectively as part of a multidisciplinary team
- Experience maintaining accurate, timely and defensible clinical records
- Experience working in areas of higher deprivation or with complex population health needs
- Experience working within a total triage or digital first model
- Experience supporting continuity of care for older patients
- Experience of home visiting as part of routine GP workload
