We are seeking an enthusiastic and motivated qualified healthcare professional (e.g. Nurse, Pharmacist, or Allied Health Professional) with a special interest in diabetes management and insulin initiation to join our friendly and supportive team. This is a fantastic opportunity to take on a well-established role, following the retirement of a valued and experienced colleague. You will play a key role in delivering high-quality patient care, with a strong focus on chronic disease management, particularly diabetes, supporting patients through treatment optimisation and insulin initiation, helping us to improve health outcomes for our patient population.
About the Role
Working closely with our Lead Practice Nurse, GP team, and wider multidisciplinary team, you will:
- Deliver a wide range of clinical clinics and patient assessments
- Lead and support chronic disease management, with a focus on diabetes care
- Support and/or initiate insulin therapy in line with local and national guidelines
- Contribute to the ongoing development of clinical services and best practice
- Promote health, wellbeing and preventative care across our patient population
- Work collaboratively across all three sites
Main duties of the job
Key Responsibilities
Your duties will include (but are not limited to):
- Managing and delivering patient care within scheduled clinics
- Monitoring and supporting patients with long-term conditions (including diabetes, asthma, COPD, CHD, hypertension and CKD)
- Supporting medication reviews and optimisation (where appropriate to professional background)
- Providing immunisations (including flu and travel vaccines, where qualified)
- Performing or supporting procedures
