An exciting opportunity has arisen for a Practice Pharmacist to join our dynamic and forward-thinking team at Brookside Group Practice. We are looking for 15-20 hours per week. The Practice Pharmacist will be responsible for support to the practice with medication reviews, queries and the delivery of the repeat prescription service. Also, part of the group that implements effective medicine management within the practice, identifying areas for improvement, and initiating and managing change. The successful candidate will ideally have a specialism in diabetes and be able to conduct diabetic reviews or be willing to work towards this.

Main duties of the job:

  • Provide a medication review and query services to patients via the telephone or face to face clinics as appropriate in the practice.
  • Ensure data is recorded so that patient care is enhanced (e.g. coding, reminders) and so that it can be retrieved in a meaningful way to help manage patient care and help with data analysis.
  • Attend and contribute to various practice meetings as requested.

The Practice serves approximately 31,000 patients from our 4 surgeries in Earley, Lower Earley, Winnersh and Wilderness Road. Due to our size, we value communication and teamwork. We, therefore, spend a lot of time investing in team meetings and communication across the organisation. We are a member of Berkshire West Clinical Commissioning Group. We recently won the HSJ (Health Services Journal) Digital Award for our innovative work we've done to refine our triage processes and model of care.

Job responsibilities:

  • Provide a medication review and query services to patients via the telephone or face to face clinics as appropriate in the practice.
  • See (where appropriate) patient with single or multiple medical problems, including patients with long term conditions, where medicine optimisation is required.
  • Provide pharmacist support to clinicians attending care home or to attend themselves as appropriate. Work with care home staff to improve safety of medicines ordering and administration.
  • Managing caseload of patients with common/minor/self-limiting ailments while working within a scope or practice and limits of competence. Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate.

Person Specification:

  • Completion of an undergraduate degree in pharmacy and registered with the General Pharmaceutical Council.
  • Good standard of education with excellent literacy and numeracy skills.
  • To hold or be willing to work towards an independent prescribing qualification.
  • Have a specialism in diabetes and able to conduct diabetic reviews.

Skills:

  • Ability to prioritise, delegate and work to deadlines
  • Effective time management
  • Have an awareness of the breadth of common acute and long term conditions that are likely to be seen in general medical practice
  • Able to use EMIS Web

Experience:

  • Minimum of 2 years' post graduate experience in pharmacy.
