Are you an experienced Support Worker looking for a rewarding outreach role where every day makes a difference? We are looking for a female Outreach Support Worker to support adults with learning disabilities and/or mental health needs in the community.

What you will be doing:

  • Providing 1:1 outreach support, helping people maintain independence, build confidence, and enjoy meaningful activities.
  • Engaging with service users who are active and enjoy being out and about - walks, social activities, and community involvement are all part of the role.

About you:

  • Manage complex needs with confidence and compassion.
  • Support adults with learning disabilities and/or mental health issues.
  • Work independently - experience in domiciliary care or lone working is highly desirable.
  • Drive and have access to your own car.
  • Communicate clearly in English - due to the needs of some service users.
  • Be comfortable supporting younger, active individuals.
