Job Responsibilities
- Provide intensive 'End to End' support for individuals who are rough sleeping with complex needs.
- Advocate and enable access to appropriate services, including housing, by collaborating with existing delivery structures and local networks.
- Work as an outreach worker to assist individuals facing complex needs such as housing issues, substance misuse, offending behaviour, mental health, and physical health challenges.
- Facilitate one-to-one and group interventions to help clients address problematic alcohol and drug use.
- Support individuals in the community to foster positive lifestyle changes that aid in their recovery.
- Build resilience and confidence in clients, equipping them with personal and social assets to meet their aspirations.
Required Skills & Qualifications
