What you'll be doing:

  • Supporting individuals with complex needs, including mental health, physical health, and substance misuse issues.
  • Assessing and managing risks to ensure safety and well-being.
  • Delivering high-quality care, support, and housing management in line with organisational policies.
  • Working collaboratively with partner agencies to provide holistic, tailored support.
  • Developing and implementing client-centred support and risk management plans, aiming for suitable move-on options within six weeks.
  • Building positive relationships, motivating progress, and encouraging recovery.
  • Managing challenging situations effectively, ensuring safety, and reporting safeguarding concerns.
  • Maintaining accurate case records, updating IT systems, and producing reports.
  • Participating in rota shifts, including evenings, weekends, and bank holidays, to ensure continuous support.
  • Assisting clients with housing benefit claims and tenancy-related administrative tasks.
