We are seeking an Advanced Nurse/ Clinical Practitioner to join our busy practice on the Eastern side of Northampton. Our ideal candidate will be an experienced prescriber with an interest in one or more long-term conditions as well as the ability to assist our GPs triage minor illness calls. Our practice is patient centric and keen to ensure patients get the best treatment to meet their needs.
Main duties of the job
- Triage patients with undifferentiated and undiagnosed problems and make an assessment of their healthcare needs.
- Undertake assessment, diagnosis, treatment, prescribing, referral and discharge as appropriate and deliver key elements of NSFs, providing public health, health protection and promotion programmes that improve health and reduce inequalities.
- Work with the clinical team to achieve maximum QoF outcomes.

Job responsibilities
Job Purpose: To provide a high-quality service to the patients of the Practice; diagnosing and prescribing treatments and drugs while working within defined procedures and policies.
Duties and Responsibilities
- Receive patients with undifferentiated and undiagnosed problems and make an assessment of their healthcare needs.
- Assessment, diagnosis, treatment, prescribing, referral and discharge as appropriate and deliver key elements of NSFs, providing public health, health protection and promotion programmes that improve health and reduce inequalities.
- Ensure significant events are reported using Practice procedures.
Administration
- To maintain accurate patient records and enter onto the computer using agreed templates and Read Codes.
- To collect data and participate in regular Practice audit and research projects.
- To contribute to the preparation of any Practice development plans.
General
- To maintain own 3 yearly registration with the NMC.
- To adhere to the NMC Code of Professional Conduct.
- To maintain a safe and therapeutic environment for all Primary Health Care Team staff, patients, carers and co‑workers from other agencies.
- Ensure that all statutory regulations (e.g. Health & Safety, COSHH, etc) are implemented and adhered to.
- To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice.
- To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice.
- Attend and participate in staff meetings.
- Any other reasonable duties that may be required from time-to‑time.

- Nursing Degree.
- Masters in Advanced Practice.
- Accredited qualification in chronic disease management.
Experience
- Experience of working in triage.
