Brook Lane Surgery is a friendly, patient-focused GP practice serving our local community in Hampshire. We are looking for a motivated and compassionate Care Navigator to join our busy and supportive team. As a Care Navigator, you will play a vital role in ensuring patients receive the right care, at the right time, from the right service. You will be the first point of contact for patients, helping to direct them to the most appropriate clinician or service.
Main duties of the job:
- Provide a welcoming and professional first point of contact for patients (face‑to‑face and telephone)
- Assess patient needs using care navigation techniques and signpost to appropriate services
- Book appointments and manage clinician diaries effectively
- Handle patient enquiries sensitively and confidentially
- Support the smooth running of reception and administrative systems
- Liaise with clinical and external healthcare teams
- Maintain accurate patient records in line with NHS and GDPR standards
Person Specification
Essential:
- Excellent communication and interpersonal skills
- Strong organisational and multitasking abilities
