This position is offered initially as a 12-month fixed-term contract, with the potential for further 12-month extensions and/or the opportunity to progress into a permanent position. There may also be future opportunities for increased hours for candidates who would welcome additional sessions as the role and service develop.
We are looking for a motivated and compassionate Practice Nurse to join our friendly and forward-thinking general practice team for two days per week. Main duties of the job
- The successful candidate will deliver high-quality nursing care within a primary care setting, with a strong focus on chronic disease management and quality improvement activity.
Key responsibilities include:
- Leading and supporting CKD and CVD review clinics and quality improvement initiatives
- Undertaking structured long-term condition reviews, including diabetes, hypertension, asthma, COPD, and cardiovascular disease
- Delivering evidence-based patient education, preventative care, and lifestyle support
- Providing treatment room services such as wound care, dressings, injections, and other routine nursing procedures
- Supporting vaccination and immunisation programmes and cervical screening services where appropriately trained
- Maintaining accurate clinical records, coding, and recall systems
- Working collaboratively with GPs, pharmacists, and the wider practice team to deliver safe, effective patient care
- Contributing to ongoing service development, audit activity, and evolving NHS Wales primary care priorities
Person Specification
- Registered Nurse (Adult) with valid NMC registration
- Experience in primary care or community nursing
- Competence in or willingness to develop chronic disease management skills
- Ability to work autonomously across structured clinical sessions
- Strong organisational and prioritisation skills
- Understanding of safeguarding principles (adults and children)
- Commitment to evidence-based practice and quality improvement
- Experience in CKD and/or CVD management
- Cervical cytology qualification (NHSCSP accredited)
- Immunisation and vaccination training
- Experience working within NHS Wales primary care systems
- EMIS Web or equivalent clinical system experience
- Experience or interest in quality improvement methodologies (QI cycles, audit work)
- Advanced clinical assessment and risk stratification
- Strong understanding of data quality and clinical coding
- Ability to deliver structured QI-focused clinics
- Effective patient education and behaviour change communication
- Independent workload management across two clinical days per week

Working Arrangements:

Qualifications
- Registered Adult Nurse (RN) with current valid NMC registration

- Cervical Cytology qualification (NHSCSP accredited)
