The Rehabilitation Nurse (Band 5) is responsible for ensuring the effective provision of nursing care for patients in the unit. Responsibilities include:
- Adhering to the NMC Code of Professional Conduct.
- Ensuring the criteria for CPD is met to maintain nursing registration.
- Practicing according to Bromley Healthcare Policies and Procedures.
- Demonstrating active interest in at least one key area of community nursing.
- Acting in accordance with nurse prescribing guidelines if applicable.
- Participating in own appraisal and accessing clinical supervision process as agreed.
- Providing integrated patient care with other members of the multi-disciplinary team.
- Participating in team meetings and disseminating information.
Main duties include:
- Providing high quality and skilled nursing/rehabilitation care and support to patients, carers, and their families.
- Liaising with community teams, GP surgeries, statutory and voluntary agencies.
- Undertaking holistic assessments of the patient.
- Participating in multi-disciplinary activities such as case conferences and discharge planning.
- Planning, implementing, and evaluating evidence-based programmes of care.
- Ensuring accurate collection and recording of data.
- Maintaining community nursing standards as agreed by Bromley Healthcare.
The post holder is expected to demonstrate the values of Bromley Healthcare including Compassion, Health and Wellbeing, Continuous Learning and Innovation, and Belonging. The post holder must comply with all relevant policies, procedures, and guidelines.
Safeguarding responsibilities include safeguarding children, young people, and adults at risk, and adhering to Bromley Healthcare's safeguarding procedures.
Essential criteria include evidence of Continued Professional Development and NMC registrations. Desirable criteria include willingness to work towards Advanced Assessment Skills and good communication skills.

Salary is determined by NHS experience and current banding/pay point. External applicants will begin at the entry point of the salary scale in line with NHS terms and conditions.
