A Health Visitor vacancy in Bexley Borough - Band 6 £44,485 - £52,521 Inc HCAS per annum pro rata. Full time and Part time hours will be considered. The Bexley Health visiting service is looking for enthusiastic and versatile Health Visitors, who are keen to develop their leadership skills and shape the future of Public Health Nursing in Bexley. Bexley 0-19 is a caring and progressive service, which provides support to families with children aged zero to 19 years. Applicants would be responsible for managing a caseload of families, working to identify need, provide support and offer early intervention packages where needs are identified. Proven Health Visiting experience required.

Applicants would be responsible for adopting a Think Family approach to safeguarding, with the aim of keeping children safe and well. Staff are expected to utilise their embedded skills and knowledge, as well as their clinical judgement, to implement evidence-based care planning and support packages to families. Bexley is invested in the development of Health Visiting, and this is underpinned by our Public Health Nursing strategy and training programme for all roles within the PHN teams.

Main duties of the job

  • Working as part of an integrated team and leading on the delivery of the Healthy Child Programme.
  • Completing HCP mandated reviews, packages of care and safeguarding.
  • Adopting a Think Family approach when working with families.
  • Identifying health needs through an ecological assessment and in partnership with families, agreeing personalised plans to address the needs identified.
  • Delegating aspects of care to the Community Nursery Nurses and Staff Nurses within the skill mix team, where safe and appropriate.
  • Engaging and participating in service improvement.
  • Continuously developing your own skills and those of others.
  • Working in partnership with clients and other partners, assessing healthcare needs to develop, and evaluating programmes to address the health needs of the population.
  • Working in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities.

This role is community-based and so the majority of duties are completed between team bases, community settings or client homes.

Job responsibilities

  • Support collecting data to inform the performance management framework for the service ensuring team members are completing timely and accurate data collection via EMIS.
  • To liaise with the School Nursing Service, with particular reference to vulnerable children, their families and children with special needs.
  • To participate in regular clinical and safeguarding supervision.
  • To attend and participate in team meetings and 1-1 management supervision.
  • To plan and record individual programmes of care for children and families.
  • To assist in the identification of local health needs and ensure the optimum use of resources.
  • Delegate work to Community Nursery Nurses that is within the sphere of competence of the skill mix and assist in training.
  • To participate in an annual appraisal of their work where objectives will be set.
  • To undertake such training as may be required to meet the needs of the service both mandatory and service-specific.
  • To assume responsibility for maintaining own level of professional development and clinical competence.
  • All staff are responsible for identifying areas within their service which do not meet Clinical Governance quality standards.
  • All staff are expected to participate in Clinical Governance actions to achieve Clinical Governance within their service.
  • All clinicians are expected to participate in quality improvement activities including clinical audit programmes.

Person Specification

Qualifications

  • Essential: Registered Nursing or Midwifery Certificate/Diploma/Degree or equivalent.
  • Desirable: Qualified Nurse Prescriber.

Professional Registration

  • Essential: NMC registration as Nurse or midwife and Specialist Community Public Health Nurse (or currently completing SCPHN course).
  • Desirable: Experience in supervising / assessing students / holds a mentorship qualification.

Training

  • Essential: Evidence of Continuing Professional Development / Professional updating of skills.
  • Desirable: Evidence of further professional learning. Knowledge of current research pertaining to Specialist Community Public Health Nursing.

Skills

  • Essential: Current knowledge and understanding of reports/legislation affecting community nursing including current PHE directives. Teaching experience. Leadership qualities. Management skills. Knowledge of the public health role of contemporary health visiting. Experience of Safeguarding issues. Evidence based practice. Computer literate. UK Driving Licence and with use of car for work purposes.
  • Desirable: Experience in data analysis / assessing service performance in relation to KPIs. Experience in completing audits.
