Job overview: Urgent Community Response Bromley Hospital at Home Staff Nurse - Band 5 (Bank) £36,943 - £44,900 Inclusive of HCAS per annum pro rota Bank shifts 8-8pm. The service provides diagnosis, treatment and monitoring of patients in their usual place of residence using both face to face and virtual care in the London Borough of Bromley. Operating 7 days per week 8am to 8pm.

Detailed job description and main responsibilities:

  • To undertake a focussed person-centred approach, ensuring quality standard and delivery of care for patients.
  • To develop safe clinical skills in community setting to proactively manage very high intensity users of health and social care services.
  • To undertake nursing procedures as indicated, such as catheterisation, monitoring of vital signs, phlebotomy, IV therapy and cannulation in the patient’s home.
  • To proactively monitor and manage a patient caseload with sound clinical autonomous decisions about patient care.
  • To confer and work with the patient's GP and other healthcare professionals to ensure robust communication around clinical issues.
  • To sensitively deal with distressing or emotional circumstances regarding patient care.
  • To actively seek GP/Consultant to review polypharmacy and other medication issues.
  • To work effectively as part of a multidisciplinary team.
  • To empower patients and their families through appropriate support and education.
  • To utilise formal and informal reporting mechanisms to ensure effective internal and external communications.
  • To continuously challenge practice, ensuring a sound evidence base, thus meeting clinical governance requirements.
  • To contribute to partnership working with Acute Trust GPs, Nursing/Residential home staff and other health care professionals.
