Bromley Hospital at Home is an integrated service delivered through Bromley Healthcare and One Bromley partners for avoiding acute hospital attendances and admissions while also supporting patients' early discharge from acute hospital beds. The service provides diagnosis, treatment and monitoring of patients in their usual place of residence using both face to face and virtual care in the London Borough of Bromley. Operating 7 days per week 8am to 8pm. Referrals are accepted to the service from the Acute Trust - Princess Royal University Hospital (PRUH) & Bromley Healthcare Urgent Community Response - Rapid Response Team.

Main duties of the job:

  • To comply with the duties placed on employees by the Bromley Healthcare's Health and Safety Policy and Related Procedures and to act in accordance with all instruction, information and training supplied in relation to those duties.
  • Such other duties as requested, compatible with the post, which may arise from time to time and flexibility to support the services within UCR.
  • Observe confidentiality of records and health information at all times and be aware of and practice the principles of the Data Protection Act of 1984, as this applies to computerised information.
  • To undertake appropriate training to meet individual developmental needs within the role.
  • Undertake the measurement and evaluation of work and current practices as directed by team lead as part of the clinical audit programme.
  • To participate in achievement of service objectives to demonstrate productivity, performance and effectiveness.

Job responsibilities:

  • Deliver person‑centred, high‑quality care to patients in their own homes.
  • Manage a complex caseload of high‑intensity patients with frailty, comorbidities and polypharmacy.
  • Undertake community‑based clinical procedures including IV therapy, cannulation, phlebotomy, catheterisation and clinical monitoring.
  • Make autonomous clinical decisions, seeking support appropriately.
  • Work closely with GPs, Consultants and MDT colleagues to prevent hospital admission.
  • Communicate sensitively with patients and carers, including in distressing or emotional situations.
  • Support medication review and polypharmacy management.
  • Promote clinical governance, audit and evidence‑based practice.
  • Contribute to UCR performance, KPIs and escalation management.
  • Work in partnership with acute trusts, social care, care homes and voluntary services.

Person Specification:

Qualifications:
  • Evidence of continued professional development
  • Masters ECAS Course Prescribing V300
  • Professional Registration: Registered Nurse (Adults) NMC Registration
Training:
  • Respiratory/ Cardiac assessment or working towards equivalent
  • Frailty CGA assessment
Experience:
  • Acute or relevant nursing experience
  • Actively involved in clinical supervision
  • Experience of working in/with a multi‑disciplinary team
  • Knowledge of Hospital at home and Virtual wards
  • CGA Frailty assessment
Skills:
  • To have organisational skills such as delegation, prioritisation and negotiation.
  • Sound knowledge and understanding of Clinical Governance.
  • Qualified & competent to carry out male/female including suprapubic catheterisation.
  • Cannulation skills, vascular access care.
  • Experienced in the care of frailty patients.
  • Competent to administer intravenous drugs.
  • Experience of teaching & motivating others.
  • Report writing skills.
  • Experience of clinical audit.
  • Understanding of research and development.
  • An understanding of change management.
  • Specialist assessments such as continence assessments, bladder scan, ECG.
