The Integrated Respiratory Service is a specialist multidisciplinary Respiratory, Pulmonary Rehabilitation, and Home Oxygen service operating as part of Bromley's Integrated Neighbourhood Teams (INTs), in line with the NHS 10 Year Health Plan's shift to neighbourhood health. The service ensures respiratory expertise is embedded within neighbourhood-based multidisciplinary working, contributing to the wider Community Health Locality Team and INT model of integrated, joined-up care and supporting proactive, person-centred care for people with COPD and other respiratory conditions.
Main duties of the job:
- Contribute clinical expertise across the Integrated Neighbourhood Team, to support shared care planning for complex and frail patients with multiple long-term conditions, including respiratory comorbidities.
- Employ advanced assessment skills for respiratory and oxygen-related needs within the Integrated Neighbourhood Team, supporting a 'one professional down the path' approach and reducing duplication for patients.
- Mentor and upskill generalist Integrated Neighbourhood Team colleagues, including community nurses and care coordinators, in respiratory red flags.
- Develop and manage the Home Oxygen Assessment and Review Service (HOS-AR), ensuring home oxygen is appropriately assessed, provided, reviewed and, where clinically appropriate, withdrawn - covering identification and referral, long-term and ambulatory oxygen assessment, follow-up home visits and clinic appointments, and withdrawal of therapy.
- Lead on delivery, management and continuing development of nurse/AHP-led specialist respiratory clinics and the Pulmonary Rehabilitation Programme, to promote optimal self-care, education and training for patients with COPD and other respiratory conditions.
- Maintain own clinical skills in respiratory assessment, including capillary blood gas testing and interpretation, nebuliser trials and oxygen review, to maximise optimal therapy and avoid unnecessary onward referral to hospital.

The post holder is responsible for the clinical and operational quality, the performance and the development of the service and carries delegated responsibility for the service budget. The post holder works in partnership with colleagues across the directorate and the wider system, aligns the service with relevant national and local priorities, reduces health inequalities, and exercises a high degree of professional and managerial autonomy in how the service is run.

Person Specification:
- Relevant clinical Degree or equivalent qualification.
- Evidence of comprehensive study or knowledge of respiratory disease management.
- Demonstrable experience in specialist respiratory care and treatment.
- Sound knowledge of NICE guidance and governance relating to service.
- Leadership or management qualification, for example ILM Level 5 or an NHS Leadership Academy programme.
- Significant senior clinical or operational leadership experience.
- Experience of managing staff, including recruitment, appraisal, performance management, employee relations and development.
- Experience of integrated multidisciplinary working across professional and organisational boundaries.
- A valid UK driving licence and access to own car for work purposes.
