Occupational Therapist - Home Pathway
Band 6 - £45,953 - £54,254 inc HCAS per annum
Permanent - Full time 37.5 hrs
The post holders will promote independence utilising rehabilitation and enablement interventions, working collaboratively with a vast range of key health and social care teams and services within the community, as well as voluntary and public sector providers, to ensure the successful delivery of the Home Pathway Service in Bromley. Your role will encompass completing complex assessment and treatment plans, developing and leading education and treatment, and occupational therapy service development. The post will primarily involve working with elderly patients and their families with a focus on goal-orientated treatment planning within their home.
Main duties of the job:
- Supervision of junior staff
- Individual caseload management and supervision of team caseload management
- Completing return to works and managing sickness as per the trust’s policy
- Carring out detailed and timely appraisals in keeping with KPI’s
- Co-lead on the coordination of all discharges from the Home Pathway Team
- Lead on therapy input in the weekly consultant led MDT
- Streamline processes to support efficiency within the Home Pathway
- Service development; a review of current Occupational Therapy services and an innovative approach to developing new aspects of the service
Person specification:
- Degree in Occupational Therapy
- Professional Registration
- HCPC Registration
- Member of the Royal College of Occupational Therapists
- Evidence of Continuing Professional Development
- Moving and Handling and Specialist Seating expertise
- Knowledge and application of Occupational Therapy assessments and intervention relevant to client group
- Able to work autonomously and set own priorities
- Understanding of clinical governance
- Evidence of contributing to Audit and evaluation
- Post qualification experience in a range of clinical settings
- Experience of multiprofessional working
- Supervision of junior staff and students
- Full driving licence & Access to a car
- Quality Improvement experience
