As a Community Nursing Associate, you’ll become a valued member of our dynamic Community Team, playing a vital role in delivering high-quality, compassionate nursing care to individuals in their own homes and clinic settings. You’ll help support patients’ physical, psychological, social, and public health needs — ensuring their unique story, background and circumstances remain at the heart of every decision. With the right competencies, you will take delegated responsibility for a pre‑agreed caseload, working under the supervision of an experienced Registered Nurse.

Main duties of the job:

  • The Community Nursing Associate will be a member of a Primary Care Network (PCN) Team mainly focusing on bridging the gap between healthcare assistants and registered nurses.
  • You will work closely with our multidisciplinary team to deliver safe, effective, and person-centred care to patients.
  • You are required to act within your professional boundaries, supporting and working alongside a multi‑disciplinary team within general practice.
  • The post holder will contribute to providing nursing care to patients registered in the GP practice.
  • Nursing Associates actively contribute to a patient’s health and wellbeing through the delivery of holistic care.

Qualifications:

  • Registered Nursing Associate on the NMC register
  • GCSE Grade A‑C in Maths & English or skills level 2 Maths & English or equivalent

Knowledge:

  • Understands the importance of following procedures and treatment plans
  • Understands the scope of the nursing associate

Experience:

  • Community experience
  • Experience of working in teams

Skills:

  • Ability to develop effective and appropriate relationships with people, their families, carers and colleagues
  • Ability to undertake and understand physiological measurements and observations
  • To strive to improve the service you provide, suggesting changes to practice in line with best practice guidance
