As a Community Nursing Associate, you’ll become a valued member of our dynamic Community Team, playing a vital role in delivering high-quality, compassionate nursing care to individuals in their own homes and clinic settings. You’ll help support patients’ physical, psychological, social, and public health needs, ensuring their unique story, background and circumstances remain at the heart of every decision.
Main duties of the job:
- The Community Nursing Associate will be a member of a Primary Care Network (PCN) Team mainly focusing on bridging the gap between healthcare assistants and registered nurses.
- You will work closely with our multidisciplinary team to deliver safe, effective, and person-centred care to patients.
- You are required to act within your professional boundaries, supporting and working alongside a multi-disciplinary team within general practice.
- The post holder will contribute to providing nursing care to patients registered in the GP practice.
- Nursing Associates actively contribute to a patient’s health and wellbeing through the delivery of holistic care.
Qualifications:
- Registered Nursing Associate on the NMC register
- GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent
Knowledge:
- Understands the importance of following procedures and treatment plans
- Understands the scope of the nursing associate
Experience:
- Community experience
- Experience of working in teams
Skills:
- Ability to develop effective and appropriate relationships with people, their families, carers and colleagues
- Ability to undertake and understand physiological measurements and observations
- To strive to improve the service you provide, suggesting changes to practice in line with best practice guidance
