- The Bromley Health Visiting Service is looking for enthusiastic and versatile Health Visitors who are keen to develop their leadership skills and shape the future of Public Health Nursing in Bromley.
Applicants will manage a caseload of families, identify needs, provide support and offer early intervention packages where needed.
Main duties of the job:
- Working as part of an integrated team and leading on the delivery of the Healthy Child Programme.
- Completing HCP mandated reviews, packages of care and safeguarding.
- Adopting a Think Family approach when working with families.
- Identifying health needs through an ecological assessment and agreeing personalised plans to address the needs identified.
- Delegating aspects of care to Community Nursery Nurses and Staff Nurses within the skill mix team, where safe and appropriate.
- Engaging and participating in service improvement.
- Continuously developing your own skills and those of others.
- Working in partnership with clients and other partners, assessing healthcare needs and evaluating programmes to address the health needs of the population.
- Working in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities.
Full and part-time opportunities range from 22.5 to 37.5 hours per week. The role is community-based – most duties are completed between team bases, community settings or client homes.
Detailed responsibilities:
- Support collecting data to inform the performance management framework for the service ensuring team members are completing timely and accurate data collection via EMIS.
- Liaise with the School Nursing Service, with particular reference to vulnerable children, their families and children with special needs.
- Participate in regular clinical and safeguarding supervision.
- Attend and participate in team meetings and 1-1 management supervision.
- Plan and record individual programmes of care for children and families.
- Assist in the identification of local health needs and ensure the optimum use of resources.
- Delegate work to Community Nursery Nurses that is within the competence of the skill mix and assist in training.
- Participate in an annual appraisal where objectives will be set.
- Undergo required training to meet service needs.
- Assume responsibility for maintaining own professional development and clinical competence.
- Identify areas within the service which do not meet Clinical Governance quality standards and participate in action plans.
- Engage in quality improvement activities including clinical audit programmes.
Person specification:
- Evidence of Continuing Professional Development.
- Experience delivering educational packages to parents/carers.
- Current NMC registration.
- Degree/Diploma in Nursing or Midwifery.
- BSc or PGDip in Specialist Community Public Health Nursing (Health Visiting).
- Qualified Nurse Prescriber.
- Ability to autonomously manage a complex caseload.
