The Head of Borough-wide Services will provide strategic and operational leadership for specialised community health services that operate across the whole of Bromley. The postholder will maintain clinical excellence and specialist expertise that complements and supports the four Community Health Locality Teams (CHLTs) and Integrated Neighbourhood Teams (INTs). As the single accountable leader for borough-wide specialist services, this role is responsible for delivering the NHS 10 Year Plan objectives through expanded community-based specialist care that prevents hospital admission, reduces health inequalities and supports the shift from hospital to community.
This role requires expertise across multiple clinical specialties and rehabilitation disciplines. The postholder will be responsible for driving forward and innovating robust evidence-based practice in the delivery and development of specialist services, maintaining borough-wide provision that interfaces with all four Integrated Neighbourhood Teams through designated link practitioners for consistent specialist capability. The postholder will work in strategic partnership with acute hospitals, primary care networks, the local authority and voluntary sector to deliver proactive, preventative and community-based specialist care aligned with the CHLT Service Specification and the Bromley Healthcare Community First strategy.
The postholder will be accountable for delivering high-quality specialist community services that meet national policy requirements including the Community Rehabilitation and Reablement Model, NHS England Long Term Conditions Framework, national respiratory and diabetes standards, and the NHS 10 Year Plan’s three foundational shifts: hospital to community, analogue to digital, and sickness to prevention. The postholder will maximise digital innovation including remote monitoring, virtual consultations and patient self-management tools to extend reach and improve outcomes.
Qualifications:
- Masters degree-level qualification or equivalent senior management experience in a relevant field
- Current NMC registration as a Registered Nurse or HCPC registration as an Allied Health Professional
Experience:
- Significant senior leadership and management experience at Band 8B or equivalent in community or acute healthcare settings
- Substantial experience of leading multi-disciplinary teams delivering specialist community health services across nursing and/or AHP disciplines
- Experience of working in community health settings delivering care in patients’ homes, clinics and bed-based rehabilitation units
- Experience of partnership working with acute hospitals, primary care, local authorities and Integrated Care Systems
- Experience of service redesign, pathway integration and change management in community healthcare settings
- Experience of performance management and delivery against contractual KPIs
- Experience of specialist rehabilitation, respiratory, diabetes or neurological service development and transformation
Specific Skills:
- Excellent leadership skills including negotiating and influencing
- Competent in using electronic records and understanding of audit opportunities/processes
- Ability to provide and receive highly complex, sensitive or contentious information and communicate appropriately with service users and their families, senior managers and external agencies
- An understanding of current therapy and specialist nursing issues across rehabilitation, respiratory, diabetes and neurological services
Information Technology:
- Ability to synthesise complex data from multiple sources, interrogate information systems and databases, and use Microsoft Office applications (Teams, Excel, Word, PowerPoint) to produce high-quality analytical reports, business cases and presentations for senior leadership and external stakeholders
- Experience using EMIS or equivalent clinical system
