The Rehabilitation Nurse (Band 5) is responsible for ensuring the effective provision of nursing care for patients in the unit. The post holder will manage their own caseload and make onward referrals to appropriate services following an assessment of the patient’s needs.

Main duties of the job:
  • To provide high quality and skilled nursing/rehabilitation care and support to patients, carers and their families of a defined caseload.
  • Liaise with community teams, GP surgeries, statutory and voluntary agencies in the provision of care to facilitate a safe discharge.
  • To undertake holistic assessments of the patient, using appropriate documentation according to Bromley Healthcare’s Policies and Procedures.
  • To participate in multi-disciplinary activities such as case conferences, joint assessments, goal setting, team meetings and discharge planning.
  • To plan, implement and evaluate evidence-based programmes of care.
  • Ensure that all electronic and manual data is collected/recorded in an accurate and timely manner.
  • To work and maintain community nursing standards as agreed by Bromley Healthcare.
  • To collaborate with others in the planning and implementation of health promotional strategies and activities.
