The Rehabilitation Nurse (Band 5) is responsible for ensuring the effective provision of nursing care for patients in the unit. The role includes:
- Providing high quality and skilled nursing/rehabilitation care and support to patients, carers, and their families.
- Liaising with community teams, GP surgeries, statutory and voluntary agencies to facilitate safe discharge.
- Undertaking holistic assessments of patients using appropriate documentation.
- Participating in multi-disciplinary activities such as case conferences, joint assessments, goal setting, team meetings, and discharge planning.
- Planning, implementing, and evaluating evidence-based programmes of care.
- Ensuring accurate and timely collection/recording of data.
- Maintaining community nursing standards and participating in education and peer audit.
- Collaborating in the planning and implementation of health promotional strategies and activities.
Applicants should have:
- NMC Registration RN (A)
- Evidence of continued professional development
- Community work experience
- Experience of working in/with a multi-disciplinary team
- Recent experience of administering intravenous drugs
- Organisational skills such as delegation, prioritisation, and negotiation
