We are excited to launch our new Children and Young People Asthma Integrated Neighbourhood Team and are seeking enthusiastic, motivated and forward-thinking practitioners to join us. This is a unique opportunity to play a pivotal role in delivering high-quality, proactive asthma care across neighbourhoods, bringing together healthcare professionals, schools, families and community partners to improve outcomes and reduce health inequalities.
Main duties of the job:
- Deliver specialist asthma assessment, education and management for children and young people
- Manage a caseload of patients with complex respiratory needs
- Support asthma reviews within schools, clinics, community venues and family homes
- Work collaboratively with GPs, paediatricians, pharmacists, school nurses, health visitors, social workers, family support services, education colleagues and the wider multidisciplinary team
- Participate in multidisciplinary team meetings and neighbourhood-based care planning discussions
- Contribute to integrated care plans that consider both health and wider social determinants of health
- Provide advice and support to professionals across health, education and social care settings
- Lead on asthma education for children, families and professionals
- Support quality improvement initiatives, population health management and service development
- Contribute to reducing health inequalities and improving access to care
- Use population health data to identify and support children at risk of poor asthma outcomes
- Act as an advocate for children, young people and families, ensuring their voice is central to care planning

Person specification:
- Qualifications: RN Child/RSCN, Evidence of specialist asthma training, Quality Improvement or Leadership qualification, Masters level or working towards
- Professional Registration: NMC – child branch
- Training: Recent study/training relevant to CYP asthma, NMC SSSA Student Assessor or equivalent, Leadership or management training
- Knowledge and Experience: Significant post registration experience in paediatrics including asthma management, Understanding of national asthma guidance, Experience of delivering education/awareness sessions to patients, families and healthcare professionals, Experience of working across multi-agency systems, Experience in service development or pathway redesign
- Specific Skills: Advanced clinical assessment skills for CYP with asthma, Ability to lead across systems and coordinate multi-agency care, Ability to work autonomously and manage complex caseloads, Ability to line manage, Good presentation and teaching skills, Good IT skills, Ability to analyse population health data, Project management skills
- Personal Qualities: Compassionate, child and family-centred approach, Ability to build relationships across systems and professional boundaries, Good planning and organisational skills
