This is an exciting opportunity to join Broadacres, working in partnership with North Yorkshire Council to deliver the LIFE contract which stands for Living Independently for Everyone. We are more than doubling the number of people we support over the coming years. You can be part of this, joining a growing support service that helps people with mental health needs and multiple disadvantages to live independently, build resilience and move towards a more stable future.
As part of the LIFE contract, you will play a key role in supporting people to sustain tenancies, prevent homelessness, develop practical life skills and access the right support at the right time. The role will involve working closely with individuals, colleagues and partner agencies to provide person-centred support across accommodation-based and floating support services.
Working closely with colleagues, health professionals, social care teams and other agencies, you'll support customers through every stage of their journey, from assessment and support planning through to resettlement and independent living.
- Deliver high-quality, person-centred support tailored to individual customer needs.
- Act as a Key Worker, developing and reviewing support plans and risk assessments.
- Promote customer independence, wellbeing and engagement within the local community.
- Recognise and respond appropriately to safeguarding concerns and risks.
- Support customers through periods of crisis and work with partner agencies to achieve positive outcomes.
- Facilitate meetings, reviews and multi-agency support arrangements.
- Encourage positive engagement, participation and the development of independent living skills.
- Work flexibly across a seven-day service, including bank holidays when required.
We are looking for people who are compassionate, resilient and committed to making a positive difference to individuals facing a range of challenges, including homelessness, substance misuse, autism, mild learning disabilities and complex needs.
- Level 2 Health & Social Care qualification (or equivalent).
- Excellent interpersonal and relationship-building skills.
- Confident IT user with experience of digital systems and record keeping.
- Good understanding of supporting vulnerable customers.
- Full UK driving licence and access to a vehicle.
- Willingness to work flexibly, including evenings, weekends and bank holidays.
- Experience supporting people with mental health needs and/or learning disabilities within a supported housing environment.
- Level 3 Health & Social Care qualification (or equivalent).
- Knowledge of needs and risk assessments.
- Knowledge of welfare benefits and support systems.
