The Role This is an exciting opportunity to join Broadacres, working in partnership with North Yorkshire Council to deliver the LIFE contract which stands for Living Independently for Everyone.
We are more than doubling the number of people we support over the coming years.
You can be part of this, joining a growing support service that helps people with mental health needs and multiple disadvantages to live independently, build resilience and move towards a more stable future.
As part of the LIFE contract, you will play a key role in supporting people to sustain tenancies, prevent homelessness, develop practical life skills and access the right support at the right time.
This role will commence on 1st October 2026 when the new service launches Explain the Role The role will involve working closely with individuals, colleagues and partner agencies to provide person-centred support across accommodation-based and floating support services.
Working closely with colleagues, health professionals, social care teams and other agencies, you'll support customers through every stage of their journey, from assessment and support planning through to resettlement and independent living.
Key Responsibilities Deliver high-quality, person-centred support tailored to individual customer needs.
Act as a Key Worker, developing and reviewing support plans and risk assessments.
Promote customer independence, wellbeing and engagement within the local community.
Recognise and respond appropriately to safeguarding concerns and risks.
Support customers through periods of crisis and work with partner agencies to achieve positive outcomes.
Complete accurate records and maintain confidential customer information.
Build strong working relationships with customers, colleagues and external professionals.
Facilitate meetings, reviews and multi-agency support arrangements.
Encourage positive engagement, participation and the development of independent living skills.
