Extra Care (Housing) Floating Support Worker, Town Close, Stokesley On a Part Time basis (18.5 hours per week) to be worked Flexibly – normally Monday, Tuesday and Friday between 9am – 5pm. Purpose of the Role:
This role involves coordinating services within the schemes (Stokesely and Northallerton), engaging in community development to ensure social inclusion for residents. The role focuses on delivering excellent customer-focused services to deliver housing support services to vulnerable older people.
Specific Responsibilities & Accountabilities:
Promote Extra Care and Sheltered Housing: Advocate for the benefits of extra care and sheltered housing, and participate in needs assessments for new applicants.
Maintain Effective Relationships: Foster effective working relationships with housing services, customer services, and other relevant internal and external individuals/agencies.
Manage Equipment: Check, test, and remove sensor monitoring and call equipment.
Promote Community Involvement: Encourage resident involvement in the local community and promote their rights.Promote Anti-Discriminatory Practice: Ensure respect and value towards all people by challenging discriminatory practices.
Welcome New Residents: Assist new residents in settling into the scheme and making friendships.
Develop Relationships: Maintain open and honest relationships with residents, responding appropriately to enquiries, concerns, and issues. Encourage resident participation in decision-making.
Enhance Independence: Provide information and develop life skills for residents to enhance their independence and quality of life, including health promotion.
Social and Learning Activities: Participate in the development and implementation of person-centred social and learning activities for residents.
Coordinate Joint Working: Assist in coordinating joint working to meet individual residents\’ needs and participate in reviews as required.
Liaise in Times of Crisis: Make necessary arrangements with relevant individuals and/or agencies during times of crisis, including terminal illness, sudden death, and end of tenancy.
Ensure Record Accuracy: Maintain the accuracy, accountability, and confidentiality of all records.
Compliance with Data Protection: Ensure all work and services comply with data protection legislation.
Respond to Emergencies: Be available to respond to emergency calls and provide cover in other schemes as required.
Job Role: The post holder will:
Report to the Extra Care Manager, ensuring the scheme meets the standards set by the Association and relevant commissioning bodies.
Deputise for the Extra Care Support Worker when required and provide cover during periods of holiday or sickness.
Provide cover at other schemes when requested.
Work outside of normal office hours, including weekends, bank holidays, and other designated holidays as required.

Hold a valid driving licence, have the ability and confidence to drive, and have access to a vehicle with appropriate insurance.
