About The Role Support Worker – Discharge to Assess

What will a day in the life of a Support Worker involve?
- Work closely with people, their families, and other professionals to create care plans tailored to their needs and goals.
- Assist and enable individuals with activities of daily living, including bathing, dressing, meal preparation, medication management, and mobility support.
- Collaborate with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
- Monitor and evaluate individuals\’ progress, adjusting care plans as needed to promote optimal recovery and well-being.
- Maintain accurate and thorough documentation of interactions, care plans, and progress.
To be a successful Support Worker, what will you need?
- Previous experience in a similar care setting, preferably within healthcare or social care, but not essential full training.
- Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals’ lives.
- Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
- Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
- Ability to work independently, demonstrating initiative and reliability.
