Support Worker – Discharge to Assess
Location:Fife, community based
Salary:£22,932 (£12.60 per hour) per annum, pro rata
Hours: 16, 24 and 35 hours available
Contract: Permanent
This is a Regulated Service and successful candidates will be required to register with the Scottish Social Security Council (SSSC) within 3 months of starting.

What will a day in the life of a Support worker involve?
  • Work closely with people, their families and other professionals to create care plans tailored to their needs and goals.
  • Assist and enabling individuals with activities of daily living, including bathing, dressing, meal preparation medication management, and mobility support.
  • Collaborate with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
  • Monitor and evaluate individuals progress, adjusting care plans as needed to promote optimal recovery and well-being.
  • Maintain accurate and thorough documentation of interactions, care plans, and progress.
To be a successful Support Worker, what will you need?
  • Previous experience in a similar care, preferably within a healthcare or social care setting but not essential full training.
  • Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals’ lives.
  • Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
  • Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
  • Ability to work independently, demonstrating initiative and reliability.
  • Flexibility to work various shifts, including evenings, weekends, and bank holidays.
