About the Role
We are looking for a passionate and enthusiastic person with great people skills to join our Health and Care service as a REACT Support Worker to provide reactive, focused and flexible support to adults to live well and reduce vulnerability, whether following a hospital stay to enable early discharge or to those living in the community.
Duties and Responsibilities
- Work within the REACT team, part of a multidisciplinary team with medical staff, occupational therapists, mental health and other healthcare professionals.
- Provide support at the service user’s home to prevent hospital admission through tasks such as shopping, housework, prescription collection, respite sitting and welfare checks.
- Speak with healthcare professionals about service user requirements.
- Drive to different locations, including hospitals, doctors’ surgeries, pharmacies, shops and patient homes.
- Offer both emotional and practical support to people using our service.
- Assess service users’ future needs and help develop and create support plans, signposting to relevant agencies and community groups for support and engagement.
- Ensure systems are updated so the support offered to patients can be tracked.
Qualifications and Requirements
- Full UK manual driving licence.
- Good knowledge of NHS and social care services.
- Professional, able to handle queries in a diplomatic and confidential manner.
- Flexible and able to work outside normal hours.
- No requirement for personal care or medication administration.
