Location: Ipswich

Salary: £24,479 per annum pro-rata

Hours: 35 per week – Monday to Friday 10am to 6pm

Contract: Fixed Term Contract until 30th September 2027

Driving: Full UK driving licence and access to a vehicle (business mileage reimbursed)

About the Role

We are looking for a passionate and enthusiastic person with great people skills to join our Health and Care service as a REACT Support Worker to provide reactive, focused and flexible support to adults to live well and reduce vulnerability, whether following a hospital stay to enable early discharge or to those living in the community.

Duties and Responsibilities

  • Work within the REACT team, part of a multidisciplinary team with medical staff, occupational therapists, mental health and other healthcare professionals.
  • Provide support at the service user’s home to prevent hospital admission through tasks such as shopping, housework, prescription collection, respite sitting and welfare checks.
  • Speak with healthcare professionals about service user requirements.
  • Drive to different locations, including hospitals, doctors’ surgeries, pharmacies, shops and patient homes.
  • Offer both emotional and practical support to people using our service.
  • Assess service users’ future needs and help develop and create support plans, signposting to relevant agencies and community groups for support and engagement.
  • Ensure systems are updated so the support offered to patients can be tracked.

Qualifications and Requirements

  • Full UK manual driving licence.
  • Good knowledge of NHS and social care services.
  • Professional, able to handle queries in a diplomatic and confidential manner.
  • Flexible and able to work outside normal hours.
  • No requirement for personal care or medication administration.
