Location: Covering Kensington & Chelsea, London
Salary: £30,000 per annum (plus ILW, if residing & working in London)

Join our pioneering NHS programme, High Intensity Use (HIU) Programme as an Outreach Caseworker, supporting vulnerable individuals across Kensington & Chelsea. You’ll support individuals who frequently access services, empowering them to take control, avoid crisis, and reconnect with their communities.
What does a typical day as an Outreach Caseworker involve?
- Supporting individuals with complex needs to access the right help and avoid crisis.
- Identifying social factors driving high use of emergency services.
- Building trust with people facing challenges like trauma, addiction, and mental health issues.
- Reducing stigma and shifting behaviours through community engagement.
- Working closely with partners like A&E, GPs, housing teams, and mental health services.
What will you bring to the role of Outreach Caseworker?
- Experience supporting vulnerable people with complex needs, e.g. substance abuse, mental health conditions.
- Strong communication skills and the ability to build trust.
- Confidence in delivering casework, person-centred planning and risk assessment.
- Quick thinking and effective negotiation in fast-paced settings.
- Accurate record-keeping and attention to detail.
