About The Role: Outreach Caseworker
Location: East Kent
Hours: per week

Are you a highly experienced HIU Lead in East Kent interested in working on an innovative NHS programme (High Intensity Use (HIU) Programme)?
The programme aims to change perceptions of those who overuse healthcare services. The team helps prevent crises, empowering individuals to self-manage and connect with their community.
Responsibilities of an Outreach Caseworker:
- Working in the community to identify and address unmet social needs that may worsen physical and/or mental health, and high emergency care usage.
- Supporting people with complex needs, including substance misuse, mental health issues, long-term conditions, and trauma, many of whom may be suspicious of formal support. Your experience will guide them to appropriate help.
- Reducing stigma and changing community mindsets by building strong relationships with partners like A&E departments, mental health teams, GP surgeries, and housing services.
What you will bring to the role:
- Experience in delivering casework with vulnerable populations, including issues like substance misuse and mental health.
- Support for individuals with complex needs, demonstrating authenticity, relatability, and care.
- Skills in person-centred support planning and dynamic risk assessment.
- Strong negotiation skills and adaptability.
- Meticulous record-keeping.
- Established links with multi-disciplinary teams, community assets, and statutory services in East Kent.
