The aim of the programme is to change perceptions of people who access healthcare more than expected. The team helps prevent crises, empowering individuals to better self-manage and connect with their surroundings.
Role of an Outreach Caseworker:
- Work in the community to identify and address unmet social needs that may worsen physical and/or mental health, including causes of high emergency care usage.
- Assist individuals with complex issues such as substance misuse, mental health, multiple long-term conditions, and traumatic experiences, often mistrustful of formal support. Your expertise will guide them to appropriate help.
- Help reduce stigma and change community mindsets by building strong relationships with partners like A&E departments, mental health teams, GP surgeries, and housing services.
Qualifications and Experience:
- Experience in casework with vulnerable populations, including substance misuse, mental health, and physical health conditions.
- Support for people with complex needs, demonstrating authenticity, relatability, and care. Ability to anticipate and resolve risks and issues.
- Experience in person-centred support planning and dynamic risk assessment.
- Strong negotiation skills and adaptability.
- Meticulous record-keeping skills.
- Established links with multidisciplinary teams, community assets, and statutory services in East Kent.
