You'll work closely with hospital teams and partner organisations to help people return home safely after a hospital stay and maintain their independence.
Our support workers are a friendly face to someone who may be struggling to do things for themselves due to age, illness, or vulnerability. What You'll Do
- Respond to referrals and complete welfare calls and home visits.
- Referrals to take the patient home from hospital to enable their discharge.
- Provide assistance to a person returning from hospital or medical facility and those identified by local health professionals as being at risk of a further decline in health or without a support mechanism.
- Attend hospital wards and discharge units to support patients.
- Provide practical and emotional support to aid recovery and independence.
- Assess service users' needs, complete and follow support plans, identify safeguarding concerns, and accurately record case notes.
- Signpost individuals to appropriate services and community resources.
This is a hands-on, community-based role where no two days are the same. You'll need flexibility and confidence working in different locations across Hull.
To be a successful Support Worker, you'll:
- Have a full UK manual driving licence and access to a vehicle.
- Have a good knowledge of services provided by the NHS and Social Care.
- Contribute to the ongoing improvement of service quality and user outcomes.
- Be professional. You can deal with queries in a diplomatic and confidential manner.
- Be willing to work flexible hours to meet service needs.
