Do you want to make a real difference at a critical moment in someone's life? We are looking for a dedicated and empathetic Mental Health Support Worker (Older Adults) to support people aged 65 and over for a four week period as they transition from hospital back into the community. This rewarding role focuses on promoting emotional wellbeing, independence and confidence after discharge, working alongside individuals at a time when understanding, reassurance, and practical mental health support can have a lasting impact.

Candidates should be based in the local area as the role will support service delivery across the four Shropshire and Telford & Wrekin Community Mental Health Service Area Teams: North Shropshire, South Shropshire, Central Shropshire (Shrewsbury East, West and Rural), and Telford & Wrekin.

What will a day in the life of a Mental Health Support Worker involve?

  • Providing practical and emotional support to older adults, including welfare calls, shopping support, home visits and medication prompts.
  • Supporting older adults following a hospital stay, helping them to recover safely and confidently at home.
  • Responding to referrals from clinicians and community health professionals, ensuring timely and person-centred support.
  • Working collaboratively to enable older adults to access community opportunities that enhance wellbeing, promote independence and reduce social isolation.

To be a successful Mental Health Support Worker, what will you need?

  • A warm, calm and patient approach, with strong people skills and a genuine commitment to supporting older adults.
  • The ability to build trusting relationships and work sensitively with older adults who may experience mental health challenges.
  • A full UK driving licence.
  • Basic IT skills, with a clear commitment to accurate record-keeping and following care plans, policies and procedures.
