Job Summary

About the role

  • Enhance End of Life Care by providing clinical expertise and advice, working in partnership across organisational boundaries to ensure care is built around the person and their family to provide timely care in the person’s preferred place.
  • Support the delivery of clinically effective, efficient and high-quality care to people with complex specialist palliative care needs, ensuring that this care is both evidence-based and reflects current best practice.
  • Using specialist skills and knowledge you will manage a caseload of patients and their families/carers with complex needs associated with living with a life challenging illness in a community setting.
  • Act as a specialist resource across the whole system using a team approach to influence patient care through the indirect role of providing specialist education and training to multi-professional staff/students, statutory and voluntary agencies, undertaking research, audits and service development within palliative and end of life care.
  • Joint management of the team.
  • Participate in a 7-day rota.
  • Provide expertise and advanced skills to facilitate and promote the individualised care of people with palliative and end of life care needs across Brighton and Hove and the Havens.
  • Assess, review, support and ensure the safe and effective case management of patients with complex palliative and end of life care needs across a variety of care settings within the community.
  • Offer continuing support and advice to family and carers and provide palliative and end of life care nursing advice to colleagues in both primary and secondary care settings.
  • Link with staff across all providers to promote fully integrated team working.
  • Participate in the promotion of patient‑centred care, establishing and maintaining a supportive relationship with the patient and their family.
  • Advocate for people as they approach their end of life, ensuring that their voice is heard by the promotion and support of advance care planning.
  • Ensure that care is planned around the patient and family and individualised to meet the complexity of need.
  • Receive and lead handover of patient information and status with relevant colleagues, to ensure that a seamless service is experienced by patients and their families.
