Bristol City Council is recruiting a Social Care Practitioner (SCP) for the Young Adults Transitions Service, supporting 14-25 year olds moving from children’s to adult services.

You will join a multidisciplinary team and shape care planning, Care Act assessments and independent living support across the city.

The role requires delivering support conversations, coordinating with partners, and travel across the city, with a minimum two days per week in the office.
