Senior Practitioner – Inner City & East Community Adult Social Care
Full Time, Permanent
We are looking to recruit a Senior Practitioner based within the Inner City & East Community adult social care team.
This is a great opportunity to develop your leadership and supervisory skills working in the diverse inner city area of Bristol and be part of a new locality model of service delivery.
Key Responsibilities
- Work with adults with physical disabilities, mental health conditions, learning disabilities, older adults and carers.
- Undertake Care Act assessments and planned and unplanned reviews.
- Undertake S42 (Care Act) safeguarding enquiries.
- Maximise use of social capital, reablement support and technology‑enabled care to conduct support conversations, reassess when required and update support plans.
- Support and advise practitioners in weekly ward‑round/bed‑management meetings, CPA meetings, Blue Light meetings and other family and multi‑disciplinary meetings including Continuing Health Care assessments.
The work can be pressured and challenging. A creative approach and confidence in the ‘3‑Tier’ model are required to support the team in finding strengths‑based solutions and innovative technological solutions.
You will receive support from the Management Team and Service Manager, and you must have a keen interest and experience in multi‑disciplinary work, the ability to work quickly and proportionately, confident decision‑making and positive risk‑taking.
The Senior Practitioner role involves supervising social care practitioners and social workers, overseeing case management and supporting the development of ASYEs & student social workers. You will be part of the leadership team, undertake strategic work, and build relationships with internal and external partners.
Good communication and excellent time‑management skills are essential.
