We are looking for an experienced care or reablement worker to join our city‑wide Reablement Service. The role supports people to leave hospital earlier following an illness or fall, aiming to promote confidence and independence over a period of up to 42 days.

Key Responsibilities

  • Deliver person‑centred support to vulnerable adults in their own homes.
  • Supervise reablement support assistants and manage the care element of the service.
  • Complete assessments, set and review goals with service users.
  • Use and support the use of technology, including data entry.
  • Work flexibly to respond to changing needs as part of a team and independently.

Key Qualifications and Requirements

  • Experience providing care to vulnerable adults in the community.
  • NVQ/QCF Level 3 or equivalent qualification, or willingness to undertake training in‑role.
  • Effective communication skills and ability to supervise and motivate staff.
  • Valid driving licence, access to a motorised vehicle, willingness to obtain vehicle business insurance.
  • Compliance with Bristol Clean‑Air‑Zone regulations.
  • Enhanced DBS check required.
