Practice Nurse

We are looking to recruit a motivated, patient-focused Practice Nurse to join our friendly and supportive multidisciplinary team. This is an excellent opportunity for a nurse who is passionate about high-quality primary care and wants to work in a well-organised, forward-thinking GP practice. The successful candidate will play a key role in delivering evidence-based nursing care, supporting long-term condition management, and contributing to preventative health services for our diverse patient population.

Main duties of the job:

• Chronic disease management (e.g. diabetes, asthma, COPD, hypertension)
• Immunisations and vaccinations
• Cervical screening
• Wound care and dressings
• Health promotion and patient education
• Supporting QOF and enhanced services delivery
• Working collaboratively with GPs, HCAs, pharmacists and the wider PCN team
• Carrying out relevant checks such as fridge temperature checks and compliance with Infection Control procedures

Job responsibilities:

Clinical Responsibilities:

• Provide safe, effective, and patient-centred nursing care in a primary care setting
• Undertake long-term condition management clinics including (but not limited to) diabetes, asthma, COPD, hypertension, and cardiovascular disease
• Perform cervical screening in line with the NHS Cervical Screening Programme
• Administer childhood and adult immunisations and vaccinations
• Carry out wound care, dressings, and minor injury management
• Support health promotion and disease prevention activities, including lifestyle advice and patient education
• Identify and elevate safeguarding concerns in line with practice policies
• Maintain accurate, contemporaneous clinical records using EMIS Web

Professional Responsibilities:

• Work within the NMC Code of Professional Conduct at all times
• Maintain up-to-date professional registration, revalidation, and CPD
• Participate in clinical supervision, appraisal, and personal development planning
• Actively engage in training, reflective practice, and continuous learning
• Support students, new starters, and colleagues as appropriate

Quality, Governance, and Safety:

• Contribute to QOF, enhanced services, and local incentive schemes
• Participate in clinical audit, quality improvement, and service evaluation
