How You Will Contribute
- Coordinate with the assigned nurse to provide safe patient care and report on the patient's condition.
- Perform personal care, take vitals, communicate with families, and other duties as assigned.
- Travel between patients and work as part of an interdisciplinary team supporting families and patients during their end-of-life journey.

Qualifications
- Meet state training requirements
- Minimum six months experience providing personal care
- At least 18 years old
- Complete 75 hours of classroom and supervised practical training (16 hours prior to practical training)
- Current CPR certification
- Ability to read and follow written instructions and document care
- Understanding of hospice philosophy and comfort with caring for the terminally ill
- Ability to work with minimal supervision
- Ability to handle death/dying
- Strong communication and organizational skills
