SUMMARY OF POSITION

Must have 1 year of Acute care within a clinic type setting.

The Registered Nurse Primary care clinics is responsible for the delivery of patient care through the nursing process of assessment, diagnosis, planning, implementation and evaluation.

Responsible for directing, coordinating all nursing care based on established clinical nursing practices.

Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.

ESSENTIAL FUNCTIONS

  • Develops, implements and evaluates the nursing care regimens for clients:
  • Plans, coordinates and assesses client care
  • Identifies client needs and provides direct nursing care based on the client’s care plan
  • Collaborates with interdisciplinary team members to develop, implement and evaluate client care
  • Participates in the Primary Care Quality Improvement program
  • Assists with Quality Improvement audits as assigned
  • Identifies quality issues and opportunities for improving quality of care
  • Documents client care with accurate and accessible records
  • Distributes medication to clients and keeps accurate records for narcotics and controlled drugs in accordance with law and agency policies and procedures
  • Orders adequate supplies and materials for rendering nursing care.
  • Communicates with Pharmacy, Laboratories and other vendors as needed.
  • Interprets philosophy and objectives of the Primary Care nursing component to clients, families, staff and the community.
  • Maintains professional competence through participation in staff development and educational program.
  • Manages the flow of clients throughout the clinic and assists providers in treatment / management of their clients

Qualifications

  • 1 year of Acute care experience within a clinic type setting
  • EMR experience
