An opportunity has arisen for a Care Manager for 29 hours per week within the enhanced duty team of the Specialist Older Mental Health Service (SOAMHS) based at Mill View Hospital in Hove. The post holder will be employed by Brighton and Hove City Council but will be co-located within Sussex Partnership NHS Foundation Trust (SPFT) as part of an integrated service. The operating hours of the service are 9-7 Monday to Sunday. There may be a requirement to work out of hours and at weekends on a rota system for which enhancements will be paid.
About the role:
The enhanced duty team offers intensive assessment and support to people with mental health needs. It is a multi-disciplinary team and is part of the wider Specialist Older Adults Mental Health Service (SOAMHS). There may be an expectation for you to undertake work as part of the SOAMHS team.
We are looking for someone who has high professional standards with an excellent knowledge of current legislation such as the Care Act and Mental Capacity Act. You will need to demonstrate knowledge in the areas of strength-based practice and social models of support.
As a Care Manager, you will support the delivery of statutory duties placed upon us by the Care Act. The main duties of this role will be to undertake assessments, reviews, professional interventions and other duties relating to individuals, families and carers. You will be required to work collaboratively with all partner agencies to improve outcomes and promote wellbeing.
You will receive regular supervision and support and have opportunities for ongoing development and specialist training. The post holder will be responsible for:
- Assessment under the relevant legislation such as the Care Act and The Mental Capacity Act
- Providing a specialist view and/or intervention in the assessment and management of risk
The successful candidate will:
- Be able to work successfully alongside multidisciplinary colleagues.
- Have an understanding of the importance of information and advice in supporting people to maintain their own independence and wellbeing.
