Brighton & Hove City Council is seeking a motivated Social Worker to join the Hospital Discharge and Response Team (DART).

The role focuses on coordinating assessments, reviews and discharge planning across multiple sites in the city.

You will work closely with health colleagues in a fast-paced integrated service, performing Care Act and MCA assessments and safeguarding tasks to promote safe, timely discharges and positive outcomes for service users.
