About the role
Change Practitioners support children and families to address concerns that have led to the need for family help, child protection or children in care support. They build relationships, deliver specific time‑limited interventions such as group work and parenting interventions, and address specific risks like domestic abuse.
Responsibilities
- Build and maintain relational links with families and community groups.
- Deliver interventions and support families to access appropriate services.
- Contribute to family plans, review progress, and provide input on risk management.
- Participate in high quality supervision, reflective practice and training sessions.
- Take part in developing innovative teams focused on prevention and community working.
Qualifications and experience
- Experience supporting children and families in a relational approach.
