We are looking for an enthusiastic and compassionate ADHD Social Prescriber to join our innovative ADHD Primary Care Connect Service in Brighton & Hove. This is an exciting opportunity to provide short-term, person-centred support to adults newly diagnosed with ADHD, helping them develop practical strategies, improve day-to-day functioning and access appropriate community support.

Working as part of a multidisciplinary team, you will provide tailored one-to-one support for up to six weeks, empowering individuals to build confidence, increase independence and make the most of local services and resources. This is a fixed-term pilot, with the possibility of extension subject to future funding.

Main duties of the job

  • Provide structured one-to-one support for adults following an ADHD diagnosis.
  • Co-produce personalised support plans focusing on organisation, routines, emotional wellbeing, employment, education and social connection.
  • Use coaching and strengths-based approaches to build sustainable self-management skills.
  • Connect individuals with local community organisations, peer support, employment services and wellbeing resources.
  • Manage a caseload of clients receiving short-term interventions.
  • Maintain accurate clinical records and outcome data.
  • Work closely with GPs, ADHD clinicians, primary care teams and voluntary sector partners.
  • Identify safeguarding concerns and escalate appropriately.
  • Contribute to the ongoing development and evaluation of the ADHD Primary Care Connect Service.

Job responsibilities

The successful candidate will provide personalised, strengths-based support to adults recently diagnosed with ADHD, helping them adjust following diagnosis and develop practical coping strategies. Responsibilities include:

  • Delivering one-to-one support for up to six weeks.
  • Supporting individuals to improve organisation, time management and daily routines.
  • Helping people understand their diagnosis and identify practical adjustments.
  • Signposting and connecting individuals to local community resources and voluntary organisations.
  • Supporting referrals into longer-term services where appropriate.
  • Managing an allocated caseload and maintaining accurate records.
  • Working collaboratively with healthcare professionals and community partners.
  • Contributing to service evaluation and ongoing development.
  • Following safeguarding procedures and recognising when clinical escalation is required.

Person Specification

Qualifications

  • GCSE (or equivalent) in English and Maths.
  • Educated to Level 3 (A-Level or equivalent) or relevant experience within health, social care or community services.
  • Training in health coaching, behaviour change or brief interventions.
