About the Role
As Clinical Package Lead, you will take an important leadership role within a dedicated care package supporting a vibrant lady with an acquired brain injury in her own home and within the community.
She is outgoing, has a fantastic sense of humour and enjoys a busy social life. She loves going to the pub, shopping, visiting the cinema and theatre, and trying new experiences.
The successful Clinical Package Lead will understand that excellent care is about more than meeting clinical needs. It is also about supporting the individual to enjoy life, maintain relationships and remain as independent as possible.
Working closely with the Clinical Lead Nurse, Nurses, Personal Assistants, Support Workers, family and wider healthcare professionals, you will help ensure the package runs safely, consistently and effectively.
Key Responsibilities
The Clinical Package Lead will be responsible for supporting the day-to-day delivery of safe, effective and person-centred care.
Your responsibilities will include:
- Leading and coordinating the day-to-day delivery of person-centred care.
- Supporting staff to provide safe, consistent and high-quality care.
- Ensuring dignity, choice, independence and wellbeing remain at the heart of the package.
- Working closely with the Clinical Lead Nurse to ensure clinical care plans and interventions are followed.
- Monitoring care delivery and identifying any concerns or areas requiring improvement.
- Supporting the development, implementation and review of care plans and risk assessments.
- Promoting safe medication management and ensuring procedures are followed.
- Supporting effective infection prevention and control practices.
- Liaising with family members and healthcare professionals where appropriate.
- Providing guidance, encouragement and support to members of the care team.
- Monitoring documentation and ensuring records are accurate and up to date.
- Contributing to safeguarding and the protection of the individual.
- Supporting the individual to access the community and participate in activities she enjoys.
- Helping create a positive team culture based on respect, communication and shared values.
- Ensuring care is delivered in line with Brightening Lives policies, procedures and CQC standards.
As a Clinical Package Lead, you will play an important role in maintaining consistency across the care package and supporting the wider team to deliver excellent care.
What We're Looking For
We are looking for someone who is confident, organised and passionate about providing excellent care.
You will ideally have experience in a senior care, supervisory or team-leading role. Experience supporting individuals with complex needs would also be highly desirable.
You will need:
- Experience within health or social care.
- Previous leadership or supervisory experience.
- A good understanding of person-centred care.
- Excellent communication and organisational skills.
- Confidence leading and supporting a dedicated care team.
- A proactive and solutions-focused approach.
- The ability to build positive relationships with the individual, family and professionals.
- A genuine commitment to promoting independence, dignity and choice.
- Flexibility and reliability.
- A positive attitude and good sense of humour.
- A full UK driving license and access to your own vehicle.
