An excellent opportunity has become available for an experienced Care Coordinator to join a well-established, privately owned domiciliary care provider in Eastbourne.
Working alongside the Registered Manager, you will play a key role in ensuring the delivery of safe, high-quality, person-centred care.
This is a varied role that includes managing staff and client rotas, coordinating care packages, completing client assessments and person-centred care plans, and maintaining compliance with CQC regulations.
You will also provide support within the community when required and participate in the on-call rota.
This opportunity would suit someone with previous experience as a Care Coordinator or in a similar role within domiciliary care who enjoys building relationships, supporting care teams and delivering excellent customer service.
Requirements
- Previous experience as a Care Coordinator or similar within domiciliary care.
- Good understanding of CQC regulations and person-centred care planning.
- Experience completing client assessments and care plans.
- Strong communication, organisational and IT skills.
- Willingness to undertake care visits when required.
Responsibilities
- Supporting the Registered Manager with the day-to-day coordination of the service.
- Managing staff and client rotas to ensure the effective delivery of care.
- Completing client assessments and person-centred care plans.
- Liaising with clients, families, care staff and healthcare professionals.
- Ensuring care is delivered in line with CQC requirements.
