The Care Coordinator will oversee day-to-day care coordination, manage client cases, ensure effective communication between caregivers and clients and provide exceptional customer service.
Duties
- Care Scheduling
- Coordinate and schedule care visits for clients.
- Ensure all appointments are booked in a timely and efficient manner, based on client needs and staff availability.
- Staff Coordination
- Maintain effective communication with care staff to confirm their availability.
- Ensure accurate shift allocation and provide updates regarding any changes to client care needs.
- Client Liaison
- Communicate with clients and their families to understand care requirements.
- Resolve any concerns and ensure client expectations are consistently met.
- Record Management
- Maintain accurate and up-to-date records of care schedules, client preferences, and other relevant data.
- Ensure compliance with regulatory and organizational standards.
- Collaborate with healthcare providers to develop and implement individualised care plans.
- Monitor and follow up on care plans to ensure adherence to treatment protocols.
- Assist in administrative tasks such as scheduling, data entry, and maintaining office supplies.
