Social Worker – EIPH (Adults)
Job Description
37 hours per week, with consideration for reduced hours.
We are on an ambitious journey to achieve a truly co-productive approach to working with people and their carers. We believe this starts at the point of first contact with the department. Our \”Strengths-Based Model of Practice\” supports a multidisciplinary approach to enabling and supporting people who live with issues caused by disability, frailty, and ageing.
The successful candidate will be based within the Early Intervention and Prevention Hub, which includes short-term social work, community support workers, and the front door response in Adult Social Care. The team supports a multidisciplinary approach, focusing on adults with complex needs and cognitive decline. This service aims to achieve outcomes for individuals, maximize their potential, and minimize risks to independence, emphasizing choice and control.
Essential requirements include a recognized social work qualification and registration with the Care Council for Wales. The ability to communicate with customers through the medium of Welsh is also required.
Protecting children, young people, or adults at risk is a core responsibility of all council employees. An Enhanced Children and Adults Barred list criminal records check by the Disclosure & Barring Service (DBS) is required for this post.
