We are seeking a Social Care Discharge Coordinator to join our dynamic team within the Hospital Social Work Team. This is an exciting and rewarding opportunity to work at the heart of integrated care, ensuring individuals return home safely and promptly with the right support in place.
Key skills include being able to:
Work effectively within a multi-disciplinary team.
Prioritise and manage their workload in a fast-paced environment.
Build strong working relationships across health, social care, and third sector partners.
Apply sound judgement to discharge planning decisions.
Responsibilities will include:
Proactively identifying and coordinating patient discharges from hospital, supporting timely and safe transitions home.
Working closely with clinicians, social care teams, and third-sector partners to ensure effective discharge planning.
Using strength based "What matters" conversations and a positive risk management approach to agree on appropriate support pathways.
Contributing to the delivery of the Discharge to Recover and Assess model, ensuring the patient's voice is central to planning.
Monitoring discharge progress, maintaining accurate digital records (whiteboards and WCCIS) and escalating delays where necessary.
Supporting admission avoidance initiatives through urgent community response and virtual ward services.
