This is an exciting time to join our Integrated Cluster Network Teams. The cluster team works to develop a team around the person and their family or those important to them. The people we support can have long-term complex challenges around their physical health, mental health and emotional well-being. We aim to provide holistic support to allow individuals to live their lives as they wish to and ensure voice, choice, independence, and control in their lives.

We are looking to recruit a creative, enthusiastic and motivated part-time, permanent Therapy Assistant to join our Integrated Cluster Networks. This is an exciting opportunity to work across Speech and Language Therapy and Dietetics, while being operationally managed by an Occupational Therapist within a supportive and collaborative multidisciplinary team.

The successful post-holder will support the delivery of Speech and Language Therapy and Dietetic interventions. This will include:

  • Attending multidisciplinary team (MDT) meetings
  • Carrying out delegated tasks such as mealtime observations, communication activities, and malnutrition screening
  • Supporting therapy programmes, monitoring progress, and providing practical advice to individuals, families, and carers to promote safe eating, drinking, and adequate nutrition
  • Delivering training, contributing to service development, and completing associated administrative tasks to support the effective running of the service

The Teams consist of Social Workers, District Nurses, Occupational Therapists, Physiotherapists, Dietetics, Speech and Language Therapists, Pharmacy technicians, Community Psychiatric Nurses, and various technicians. We work closely with G.P. surgeries and third sector partner agencies to assess and support the needs of adults affected by long-term complex and adverse conditions as well as their carers and families.

We work closely with GPs and other cluster partners and utilise the Contingency Care Model to prevent unnecessary admissions to hospital and care homes, as part of our role to maximise independence and choice. The continued development of this dynamic service model will require work on widening partnership opportunities with statutory partners, the third sector, and individuals from the community.

We are keen to develop resilient community connections, groups, and services in Bridgend. The position will either be based in our office in Pyle or in Glanrhyd. We will offer focused and committed development opportunities for the successful candidate.

The ability to greet customers through the medium of Welsh is a requirement for this post. Protecting children, young people, or adults at risk is a core responsibility of all council employees. An Enhanced with Adults Barred list criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post. The council’s Hybrid Working Policy applies to this post.
